The incumbent is assigned to the Geriatrics and Extended Care (GEC) Service Line, and provides direct patient care to veterans within the Community Living Center/Palliative Care Unit. He/she performs a variety of supporting clinical duties with minimal guidance and supervision by a Registered Nurse and/or Licensed Practical Nurse. Those duties are described below, but are not limited to the following:
Performs a variety of personal care duties.
Performs! direct patient support work.
Provides for increased comfort a! nd spirit of palliative/hospice patients.
Identifies and reports data relative to complex patient situations.
Teaches patients and family members the necessity of continuing the procedures for proper health care.
Reinforces the patient care plan based on the patientÂ's condition, e.g. functioning of limited physical mobility, proper body mechanics, colostomy care, etc.
Performs nursing care duties such as:
Performs direct patient support work including personal patient care and/or patient teaching within the scope of his/her training.
Documents specific information on the patientÂ's ADL record and patient education on the computerized patient education template.
Assists with activities of daily living (feeding, bathing, dressing), lifting and turning patients.
Provides oral care, nail care (non-diabetic), hair and skin care, comfort measures, skin care related to feeding tubes, foley and condom cathet! ers and other related personal hygiene duties.
Performs range of motion exercises as appropriate.
Applies braces, splints, safety and prosthetic devices.
Transports patients, equipment and supplies as needed according to policies and procedures.
Takes care of patientÂ's non-routine care and comfort needs for psycho/social/spiritual well being, personal hygiene, physical comfort, nutrition, elimination, prevention of skin breakdown, rehabilitation, safety, etc.
Assists in the performance of diagnostic examinations by performing duties such as monitoring vital signs, preparing and positioning patient, or by performing a variety of routine diagnostic tests.
Reinforces patient teaching provided by the professional staff for patients with diabetes, common urine testing, colostomies and rehabilitation needs, as indicated by the documented plan of care.
Provides simple explanations prior to and/or during all nursing tr! eatments and procedures in a manner that will allay the patientÂ's anxi! ety.
Encourages patients in self-care activities within their individual capacities as outlined in their care plan.
Performs other duties as assigned by the supervisor or the needs of the service line.
Work Schedule:
One full-time (40 hours per week) position available; Night Shift 12:00 AM - 8:00 AM; Day Shift 7:30 AM - 4:00 PM (Day shift staff does rotational shifts from 3:30 PM - 12:00 AM or 12:00 AM to 8:00 AM). One part-time (20 hours per week) position available; rotational shifts.

. The selected candidate's qualifications will be reviewed to determine the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.
Basic Grade Requirements for GS-3:
GENERAL EXPERIENCE:
Six (6) months of any type of work that demonstrates the applicantÂ's ability to perform the work of the position, or experience that provided a familiarity with the subject matter or processes of the broad subject area of the occupation. Examples of general experience may include work obtained in outpatient clinic, home care or hospital environment.
OR
SUBSTITUTION OF EDUCATION:
One (1) year of education above the high school level (One year is the equivalent of 30 semester hours, 45 quarter hours, the equivalent in a college or university, or at least 20 hours of classroom instruction per week for approximate! ly 36 weeks in a business or technical school) with courses related to ! the occupation from an accredited institution may be substituted for the experience required at the GS-03 level. This education must have been obtained in an accredited business or technical school, junior college, college or university for which high school graduation or the equivalent is the normal prerequisite. If you are using education to qualify, you must submit transcripts with your application.
OR
COMBINATION:
An equivalent combination of experience and post high-school education that demonstrates your ability to perform the duties of the position is fully qualifying at the GS-03 level, but only one full academic year of study; or the semester hours in excess of the first 30 semester hours can be used in place of experience. If you qualify based on a combination, your experience must be documented and you must submit transcripts with your application.

GENERAL EXPERIENCE:

Six (6) months of specialized experience that equipped the applicant with the particular knowledge, skills and abilities (KSAÂ's) to perform successfully the duties of the position, and that is typically in or related to the position to be filled. To be creditable, specialized experience must have been equivalent to at least the next lower grade level. Qualifying specialized experience includes nonprofessional nursing care work in a hospital, outpatient clinic, nursing home, or other medical, nursing, or patient care facility, or in such work as that of a home health aid. !

SUBSTITUTION OF EDUCATION:

Qualifying specialized experience includes nonprofessional nursing care work in a hospital, outpatient clinic, nursing home, or other medical, nursing, or patient care facility, or in such work as that of a home health aid, performing duties such as (1) providing personal nursing care such as providing pre- and post-operative care, (2) support duties for diagnostic and technical treatment procedures, such as setting up and ! operating special medical equipment and apparatus, (3) caring for menta! lly ill patients, including observing, recording, and reporting changes in their behavior, and providing reassurance and encouragement, (4) assisting surgeons and registered nurses in operating room activities, including passing instruments, maintaining sterile conditions, and draping and positioning patients.
OR
SUBSTITUTION OF EDUCATION:

UNITED STATES CITIZENSHIP REQUIREMENT:
Nursing Assistants appointed to direct patient care positions must be a citizen of the United States (U.S.), except provided in 38 U.S.C. 7407(a) and subparagraph (2).
ENGLISH PROFICIENCY REQUIREMENT:
Nursing Assistants appointed to direct patient care positions must be proficient in spoken and written English as r! equired by 38 U.S.C. Chapter 73 or 74.
KNOWLEDGE, SKILLS AN! D ABILITIES (Rating Factors):
Applicants for both grade levels must possess training and experience that clearly demonstrate they possess the following knowledge, skills and abilities. Applicants may utililze VA Form 4676a, Â"Employee Supplemental Qualification StatementÂ".
1. Knowledge of standardized rules (clinical issues, directives, polices, handbooks, standard operating procedures, regulations, etc.) and skills to perform procedures to carry out nursing care and resolve recurring problems.
2. Knowledge of equipment terminology, drug terminology and supplies to identify and use properly and to communicate proper use with nurses, medical staff and family members.
3. Knowledge of standard medical terminology and skills to provide and gather information in patient care/family conferences with nurses, doctors, patients and family members.
4. Skills in understanding of diseases and illnesses to teach patients and family members th! e necessity to continue the procedures of proper health care.
