At the direction of the Director of Gastroenterology, participate as a member of the Total Pancreatic Auto Islet Team. Assume the responsibility to ensure the smooth and efficient delivery of health care services from the initial referral through the treatment process as the main point person for the transplant team. Independently organize health care resources at DHMC, and analyze and record data in a database relevant to the Pancreatic Program. Facilitate the education of nursing and medical personnel at DHMC and other facilities that care for our patients. Make sure that patients are pre-approved for the TPAT procedure.
Responsibilities:
- Provides care (education, coordination of testing, collection of labs, data) for patients pre and post pancreas auto islet transplant
- Coordinates the evaluation process for pre-transplant patients to increase efficiency for the patient and the hospital (testing includes: EGD/EUS, transplant labs, EKC, Echo/Nuclear stress, Pulmonary function tests, abdominal imaging, consults with surgery, gastroenterology, nutrition, pain clinic, endocrinology and psychiatry.
Works closely with social work to ensure correct insurance coverage before referral
Helps to simplify the scheduling of testing/labs for pre-transplant patients and in turn this reduces cost to the patient and hospital. (i.e. if a patient has already had immune studies in the past, I do not reorder for the transplant w/u unless warranted. The same goes for a colo/egd and other testing)
Follows up on the results of all testing done regarding pre-transplant w/u and makes decisions with the providers regarding interventions
Collaborates with providers regarding post-transplant patient’s medications, labs, and required testing (i.e. u/s, CT, MRI, etc)
Participates in developing and implementing protocols (SmartSets) for transplant patients (will continue to do this as we are always evaluating and re-evaluating our processes)
Keeps an up-to-date database of all transplant pati ents that have been evaluated, listed, considered, turned down, transplanted, etc. since 2012.
Currently building the post-transplant database
Communicates with multidisciplinary team members and patients/families in a timely manner in regard to patient care issues
Plans, orders, and coordinates scheduling of post-transplant medical procedures and follow-up appointments
Educates patients (within my scope of practice) on the pre-peri-post transplant process and procedures
Attends and prepares for monthly TPIAT patient review meeting
Serves as the consultant (“go-to personâ€) to the multidisciplinary team (secretaries, nutrition, surgeons , gastroenterology, pain clinic and psychiatry)
Triage calls from patients regarding transplantation, medical condition, appointments, etc.

Performs other duties as required or assigned.
Minimum Qualifications:
- Graduate from an accredited Nursing Program required.
- Bachelor of Science Degree in Nursing (BSN) with 3 years of clinical experience required.
- Two years of pancreatic nursing experience preferred.
- Demonstrated clinical competence in the area of organ transplantation.
Required Licensure/Certification Skills:
- Licensed Registered Nurse in New Hampshire required.
- Basic Life Support (BLS) Certificate required.
